He was predeceased by: his parents, Alex "Jackson" E. Mazika, Sr. and Florence Mazika (Ferrara). He is survived by: his wife Maureen Mazika (Vescera); his children, Gina Mazika, Alissa Labonte (Ron) and Alex Mazika III (Claudia Carvalho); and his grandchildren, Tiffany, Kevin, Amanda Coletta, Isabella, Lucas Labonte, Alex IV and Ava Mazika.
In lieu of flowers donations can be made to the Dana-Farber Cancer Institute or the Rhode Island Hospital Cancer Center.
