He was predeceased by: his wife Angela Cook (Harris); his parents, Wallace F. Sr. Cook and Gladys Cook (Berry); and his siblings, Wayne F. Cook, Madelyn Libby and Isabel Smith.
He is survived by: his children, Diane J. Cook (Marilyn Hines), Wallace F. Cook III (Brenda) and Pamela J. Coulter; his grandchildren, Beth Coulter, Rodney Coulter (Delilah Moleon), Madison Cook-Hines and Devlin Cook-Hines (Harris Cook-Hines); and his great grandchild Brantley Ayer.
