He was predeceased by: his parents, George Costa and Gertrude Costa of West Warwick. He is survived by: his wife Debra Costa; his stepson Jeremy O'Donnell (Aimee); his grandchildren, Breiana and Owen; his sister Nancy La Fountain; his nephew Kenneth La Fountain (Kimberly); his daughter Ava; his nephew Jeffrey La Fountain; and his sons, Tyler, Connor and Brady.
