He was predeceased by: his parents, Walter Hazard and Hazel Hazard (Fischer). He is survived by: his children, David Hazard Jr., Matthew Hazard and Breanna Hazard; his siblings, Thomas Hazard, Janet Misiaszek, Robert Hazard and Joyce Cooke; and his nieces and nephews, Crystal, Tommy, Jared, Evan, Jake, Brandi, Brian, Rachael, Curtis and Nathan.
