Polacek Sr. (Burrillville, Rhode Island), born in Woonsocket, Rhode Island, who passed away on July 31, 2023, leaving to mourn family and friends. He was predeceased by: his parents, John J. Polacek and Frances Polacek (Black); and his former wife Dianne B. Polacek (Sbraccia). He is survived by: his wife Ann Polacek (Viherek); his children, John F. Polacek Jr. (Jennifer) of Harrisville, Michael Polacek (Rebecca) of Glendale and Rebecca O'Bar (Michael) of Harrisville; his siblings, Richard Polacek of Harrisville and Kathleen Tupper of Harrisville; and his grandchildren, Regan and Rayna.
