Urbaniak, II (Andrea); his grandchildren, Katharine, Jack Shea, Ania and Thomas F. Urbaniak, III; his brother Henry S. Urbaniak, Jr.; and his sisters-in-law, Carol DeMeritt and Norma Smith (Edmund). He is also survived by nieces, nephews, and friends.
