He was predeceased by: his parents, Warren Horak and Marian Horak (Francis); his significant other Paula M. Horak; and his brothers, Kurt and David Horak.
He is survived by: his brother Todd Horak (Mia) of Gambrillis, Maryland; his sister-in-law Gloria Reillo of Williamsport, PA; his nephew Alex Horak; his children, Melonie Horak-Horn and Eric Horak (Kayla) of Williamsport, PA.; his grandchildren, Lily Horak of Williamsport, PA, Dominic Horn of Hughesville, PA, Madison Horn of Williamsport, PA and Alexcia Horn of Williamsport, PA.; his great grandchildren, Aria Reed of Williamsport, PA, Kinsley and Gabriella Horn of Hughesville, PA; and his pets, Kiki, Nala and King. He is also survived by his grandchildren and great-grandchildren.
Visitation will be held on Saturday, November 23rd 2024 from 1:00 PM to 2:00 PM at the Sanders Mortuary (821 Diamond St, Williamsport, PA 17701). A memorial service will be held on Saturday, November 23rd 2024 at 2:00 PM at the same location.
In lieu of flowers, memorial donations may be made in Brad's name to St. Jude's Children's Research Hospital, 501 St. Jude Place, Memphis, TN 38105.
