He was predeceased by: his parents, Frank Kulik and Jennie Kulik (Pesta); his nephew Stanley Stachacz; his brothers, Rev. Francis J. Kulik and Daniel Kulick; and his brother-in-law Joseph J. Polaski.

He is survived by: his sister Shirley Polaski of Clinton, NJ.
