He is survived by: his wife Bonnie Knopsnider Kozak; his children, Michael Kozak (Denise), Marsha Kozak and Melissa Horner (Chad); his grandchildren, Jarrod David, Zakary David, Jona Smith (Vic) and Ashley Green; his great grandchildren, Jaxon, Nixon, Monreaux, Maverick and Gentry; his brother Waide Kozak (Alberta); and his pets, Dakota, Oaklee and Middy. He is also survived by numerous nieces and nephews.
