He was predeceased by: his parents, John Lewis and Beatrice Lewis (Bowen); his brothers, Jack (Mary) and George Lewis; and his nephew David Lewis.

He is survived by: his wife Patricia Carroll; his sons, Thomas Lewis, II (Misty) of Shenango Twp and Brandon Lewis (Katie) of Shenango Twp; his grandchildren, Thomas III, Caden, Phoenix, Katana and Rogen Lewis; his stepgrandchildren, Cassie, Cody and Corianne Montgomery; his stepgreat-granddaughter Raelynn Jones; and his sister-in-law Kathleen Lewis of Akron, OH..
