He is survived by: his nieces and nephews, Kimberly Forshay (John), Tim Phelan (Diane), Jeff Phelan (Jennifer) and Kari Smith (Rick); and his great-nieces and great-nephews, Sean, Ben, Kaci Smith, Dani, Amy, Brad Forshay and Rori Phelan.
