She was predeceased by: her parents, Jack Hoffman and Anna Mae Hoffman (Arrigo). She is survived by: her partner Joey Chaybin; her siblings, John Hoffman of South Park, Russell Hoffman (Donna) of South Park, Janet Kunkel (Denny) of Jefferson Hills, Pennsylvania. and Gary Hoffman (Cindy) of Jefferson Hills, Pennsylvania.; her nieces and nephews, Lisa Dorfner (Don), Kevin Hoffman (Carrie), Eric Hoffman (Julie), Stephanie Dickerson (Cody) and Kristen Kunkel; and her great-nieces and great-nephews, Hunter, Ted, Lexi and Doug.

Due to Covid 19, Leslye's funeral services will be held privately.
