He was predeceased by: his father John J. Kukonik; and his stepfather Donald Smouse. He is survived by: his mother Ann Kukonik Smouse (Giannamore); his daughter Alyssa Kukonik; his siblings, Diane Montini (Jim), Daneen Joseph (Mark), John Kukonik, Mary Beth, Dawn Koch (Bob) and Dana Kudnik (Mike); and his close friend Amy Konecny. He is also survived by several nieces and nephews.
