We are sad to announce that on June 17, 2019, at the age of 82, John Richard Kizak (West Middlesex, Pennsylvania), born in Sharon, Pennsylvania passed away. Family and friends are welcome to leave their condolences on this memorial page and share them with the family.
He was predeceased by: his parents, Richard Kizak and Mary Kizak (Piai). He is survived by: his wife Gerry Zipay; his daughters, Kelly Palmer (Todd) of West Middlesex and Kimberly Underwood (Chad) of Canfield, OH; his grandchildren, Brittany Tong (Dennis), Jerrod Palmer and Linley Underwood; his great grandchild Michael Tong; and his siblings, Marcia Walters (Tom) of La Porte, IN, Donald Kizak (Patricia) of Hermitage and Michael Kizak of Transfer.
In lieu of flowers, the family suggests memorial contributions be directed to Church of the Good Shepherd, 3613 Sharon Rd., West Middlesex, Pa 16159 ; or to a charity of the donor's choice.
