He was predeceased by: his parents, Anthony Samuel and Lorene Dorothy Forberg Bolthouse; and his brother William Bolthouse. He is survived by: his children, Daniel Bolthouse, Elizabeth Bolthouse of Muskegon, MI and Sarah Buisch of Wilkes-Barre; and his brother James Bolthouse of Lansing, MI.
