She was predeceased by: her parents, Willard Fornwalt and Ethel Fornwalt (Schaeffer); her husband Charles E. Kurtz; and her siblings, Bessie Worthington, Nora Black, Harold, Henry, Paul, Carl and Marvin Fornwalt.
She is survived by: her daughter Donna K. Reeder of Watsontown; her grandchildren, Laurie Ann Spencer LaBella (Joe) of Middletown, CT and Cody Daniel Reeder of Watsontown; her great grandchildren, Kyle A. Young of Middletown, CT, Kaci M. LaBella of Middletown, CT, Ryle Jean Reeder and Macee Jean Reeder of Watsontown; and her sister Carrie Lee Fornwalt of Lewisburg. She is also survived by many nieces and nephews.
