He is survived by: his parents, William Cook and Shirley Cook (Page) of Waterford; his wife Kimberly; his children, Corwin (Alexandrea Maas), Morgan, Gabriella, Kathryn and Owen; his mother-in-law Katherine Palmer; his siblings, Holly Anderson (Dave) and Matthew Cook (Karen); and his brother-in-law Scott Palmer (Marilyn). He is also survived by many nieces and nephews, extended family and friends.

In lieu of flowers, memorials may be made to the family for the Cook children's educational expenses.
