He was predeceased by: his parents, Keith McIntosh and Olive McIntosh (Olson). He is survived by: his wife Meredith McIntosh (Hunter); his children, Keith R. McIntosh (Caroline) of Gerrardstown, W. V. and Lane E. McIntosh (Rebecca) of Charles Town, W. V.; his grandchildren, Henry, Oliver, Virginia McIntosh and Evelyn Tabb; his brother Dr. Brian McIntosh (Amy Muscaro) of Warren; and his pet Luci.
In lieu of flowers, memorial contributions may be made to the Warren County Historical Society, 210 4th Ave., Warren, PA; and to First Lutheran Church, 109 W Third Ave., Warren, PA.
