He was predeceased by: his father Stanley M. Hazen; and his sister Diane Lloyd. He is survived by: his children, Timothy Hazen (Denise) of Bullard, TX., Steven Hazen (Sara) of Cranberry Twp and Laura Hazen of Verona; and his grandchildren, Ellis, Corbin, Pierce, Skylar, Delaney, Thomas, James and Nigel.
