He was predeceased by: his parents, Steve Kozakovsky and Mae E. Knochel Kozakovsky; and his wife Susan Kozakovsky. He is survived by: his stepson David Nikolavich (Mindy) of Sarver; and his grandson Keith. He is also survived by many nieces, nephews, great-nieces and great-nephews.
