"Buddy" Forkin of Gahanna, OH, Tracy Sirney (Paul) of Valencia, PA and Brian P. Forkin (Chrisy) of Warren; her grandsons, Brian Forkin Jr. (Molly) and Aaron Forkin; her great grandchildren, Austin and Kali; and her siblings, Felix Clementi (Patty) of Warren, Vickie Surin of Jacksonville, FL and Maria Deleo of Warren. She is also survived by numerous nieces and nephews.
