He is survived by: his wife Wavalene Sparks McIntosh of Brownsville; his sons, Jeffrey P. McIntosh (Joy) of Wellsville and James C. McIntosh (Joell) of Brownsville, Pennsylvania; his grandchildren, Francesca, Brenton, Sondra, Brittney, Jamie and Jarin; his great grandchildren, Vincent and Flynn James; and his sisters, Brenda Hamilton of Michigan and Mary Thomas of Kentucky. He is also survived by other relatives and friends.
