He is survived by: his wife Shirley Blake (Smith); his daughter Michele Leanna Tate (Chad) of Camp Hill, PA; his granddaughter Madison Tate; and his brother Robert Blake (Dorothy) of Jamestown, PA. He is also survived by nieces, nephews, and cousins.
