He was predeceased by: his mother Janet Moore (Wharry); his grandparents, John, Dorothy Wharry, Bertha and Ernie Moore; his parents-in-law, Larry and Dorothy Carnahan; and his great-niece Izzy Peterson.

He is survived by: his father Jack Dennis Moore Sr.; his wife Debra Moore (Carnahan); his children, Jacob Alan Moore (Loralee Lindemuth) of Tidioute and Hayley Marie Moore of Youngsville; and his siblings, Connie Peterson (Rick) of Tidioute, Crystal Gertsch (Ted) of Warren and Jack Moore Jr. (Lisa) of Clarion. He is also survived by several nieces, nephews, great nieces and nephews and numerous friends.

In lieu of flowers, the family suggest donations be made to the Nelson Funeral Home, 314 North Main Street, Youngsville, Pa 16371, to help defray funeral expenses.
