He was predeceased by: his parents, Francis V. Kolodziejczak and Theresa M. Kolodziejczak (Ducharme); and his brother Paul E. Kolodziejczak. He is survived by: his children, Jonathan P., Nickolas D. and Stephany G. Kolodziejczak; his former wife Laura J. Kolodziejczak (Langlois); his siblings, David F. Kolodziejczak (Mary) of Dudley, Joseph H. Kolodziejczak of Copalis Beach, WA., Ann T. Gaudreau (Rick) of Dudley and Debra A. Kolodziejczak of Woodstock, CT.; his aunt Helen Russell of West Brookfield; and his child His. He is also survived by several nieces, nephews and cousins.
