She was predeceased by: her husband William L. Cook; her parents, George Sr. Suprick and Verna Labanich Suprick; and her siblings, George Jr., Michael, Verna Franko, Anna Rita Lewis, Helen Conaghan and Mary Suprick.
She is survived by: her nieces and nephews, George Suprick III (Sharon), Mark Suprick (Tina Traver), James Conaghan (Beth Ann), Raymond Franko (Fran), Catherine Conaghan, Michelle Smith (Tom Smith) and Andrea Weisenburger (John Weisenburger); her stepdaughter Carol Lee Sullivan (Paul Sullivan); her stepgranddaughter LeAnn Evans; and her stepgreat-granddaughter Leah Beth Evans.
