He was predeceased by: his parents, George W. Hlavac Sr. and Anna Hlavac (Potocheck); his siblings, George W. Hlavac Jr., Mary Saxon and Frances Casciato; and his pets, Montgomery and Cassie.
He is survived by: his wife Betty; his daughters, Cheryl Hlavac Zuczek (Stan), Nancy Donahue (George), Marybeth Hlavac, Tina Mayerski (Joseph P.), Pringle and JulieAnne Hlavac Bruno (Joseph M.); his grandchildren, Jeffrey Zuczek (Chantal), William Joseph Bruno, Sabrina Elizabeth Bruno, Jeremy Joseph Mayerski and Joshua William Mayerski; his great grandchildren, Carlie and Ellie Chan-Zuczek; and his sister Betty Kostick. He is also survived by his numerous nieces and nephews.
