He was predeceased by: his wife Sophie Corsale Fazio; his son David E.; and his brother Anthony Fazio (late Irene). He is survived by: his children, Gary J. Fazio (Sharon), Edward D. Fazio of Swisshelm Park, Mary Kathryn Costa (Alfonso) of Wellington, FL and Thomas J. Fazio; his siblings, Russell Fazio (late Rosemarie) and Dr. James Fazio (Marie); his great grandchildren, Sophie, Alfonso "Alfie", Francisco "Francis" and Charlotte Mary; and his niece Paulette. He is also survived by his nieces and nephews.

In lieu of flowers memorial contributions may be made to the American Heart Association.
