He was predeceased by: his parents, Frank Zajaczkowski and Victoria Zajaczkowski; his wife Elizabeth; and his children, Raymond and June Mahoney.
He is survived by: his daughter Susan Maille; and his siblings, Edmund Zack and Arlene Palisak. He is also survived by 7 grandchildren; 8 great-grandchildren; and numerous nieces, nephews, and cousins.
