He is survived by: his wife Amy Putnam Flaisman; his children, John Jordan of Sugar Grove, Jared Flaisman of Sugar Grove and Brianna Clever; his granddaughter Braelynn Canfield; his friend Danny Garcia; his parents-in-law, John and Barb Putnam; and his siblings, Geni Nieto (Richard) of Pulaski, PA, Theresa Wolf (Greg), Gina Flaisman (Larry) of Ambridge and Sharon Bradley (Dave) of Warren, OH.
