He was predeceased by: his sister Rose. He is survived by: his spouse Sally Smith (Hart); his daughter Ashley; and his siblings, Joseph Smith of Ephrata, Daniel Smith of Ephrata and Viola. He is also survived by many nieces, nephews, and friends.
