He is survived by: his wife Denise Bullington Sliwka; his brother Kenneth; his sister-in-law Susan; his nephew Matthew; his godson Charles; and his parents-in-law, Lester and Rosemarie. He is also survived by many aunts, uncles, cousins and friends.

Charitable contributions in his honor may be made to a charity of the donor's choice.
