He was predeceased by: his parents, Domenic Costa and Rose Costa (Calagiuri). He is survived by: his wife Ethel Costa (Van Dyke); his sons, Guy Jr. (Susan), Glenn (Marcia) and Greg; and his daughter-in-law Michele Costa. He is also survived by 6 grandchildren; and 6 great-grandchildren.
