He was predeceased by: his wife Gail Moore Church; his parents, John F. Church and Grace Anderson Church; his siblings, Bill, Howard, Ed, John Church, Wilma Phillips and Loreen Kiger; and his daughter Denise Church.
He is survived by: his children, Danny Church (Karen) of Wana, WV, Donna Tuttle (Tim) of Wana and Debra Williams (Mark) of Miracle Run, WV; his grandchildren, Kim Sullivan (Scott), Keith Church, Todd Foley (Pam), Shawn Tuttle (Colleen), Beth Jones (Eric), Tracey Wade (Jay), Ashley Hunt (Greg), Cody Tuttle (Nina Nyugen) and Adam Williams; his sister-in-law Velda Moore of Wadestown; and his caregivers, Misty, Hope, Melissa, Daniel, Erica and Kirsten. He is also survived by 15 great-grandchildren, and 4 great-great-grandchildren.
