Kostyk (South Fork, Pennsylvania), born in Johnstown, Pennsylvania, who passed away on October 7, 2021, at the age of 59, leaving to mourn family and friends. She was predeceased by: her parents, John Sr. Kostyk and Geraldine Kostyk (Fetterman). She is survived by: her siblings, John Jr., Walter, Crystal (Robert Yahnert) and Victoria (James Vasbinder). She is also survived by nieces and nephews.
