He was predeceased by: his parents, Michael Jr. Blazosky and Mary Blazosky (Chinchar); his wife Ann Marie Blazosky (Glovany); his son Mark Michael Blazosky; his grandson Michael Alberto Diaz; and his son-in-law Dr. Carlos A. Diaz.
He is survived by: his children, Susan Blazosky, Bernard (Jennifer) and Heidi Diaz; his grandchildren, Christa, Alexander, Katrina and Julia Diaz; his son Joseph Blazosky; his sisters, Phyllis Slonac (Joseph) and Marcia Costlow (Ronald); and his parents-in-law, Paul Glovany, Claire Glass (Ronald), Mary Grace Martyak (William), Joan Cammarata (Samuel) and Thresa Valko (Peter).
He was also very active in the Adams Township Area Lions Club and received the Melvin Jones Fellow honor for dedicated humanitarian service from the Lions Club International Foundation.
