She is survived by: her parent Cora Mardell Hershberger Fullem of South Connellsville; her husband Ken Urbach; her children, Jason Urbach (Renee) of Bullskin Twp, Jamie Callen and Robert Callen of Greensburg; and her sisters, Nancy D. Mauk of Oaks, PA. and Karen M. LeWellen of Bullskin Twp.
