He was predeceased by: his parents, Ziggie L. Kozlosky and Jennie M. Kozlosky (Jarosick). He is survived by: his wife Kathy A. Kozlosky (Miller); his sons, Mark A. (Karen) and Jeffrey R. (Rebecca); and his grandchildren, Ryan, Adam, Emily, Michael, Kaia and Lucas.
