He was predeceased by: his parents, Joseph M. Urbanik and Mary Urbanik (Homola); his wife Edna M. Jays; and his siblings, Joe, Mike, Steve, Andrew, Mary Formich and Ann Butterworth.
He is survived by: his children, Valerie A. Etters of Clarence, Frank Urbanik, Jr (Shirley) of Clarence, Mary L. Hipple of Howard, James R. Urbanik (Ginger) of Clearfield and Daniel L. Urbanik of Tyrone; and his siblings, John Urbanik of Clarence, Helen Fomich of Clarence and Mildred Mihalik of Snow Shoe. He is also survived by 12 grandchildren and 22 great-grandchildren.
