He was predeceased by: his parent Frank Perfetta; his wife Mary Perfetta (Santucci); his son Frank Perfetta; his granddaughter Samantha; his great grandchild Jacob Ryan; his brother Frank Perfetta; and his brother-in-law Harold Shawley.

He is survived by: his parent Sophia Perfetta (Mekolajcik); his children, MaryAnn Turk (Jeff) of Slickville, Cathy Peak (David) of Bell Township, John R. Perfetta Jr. (Brenda) of Slickville and Steve Perfetta (Carrie Miller) of Slickville; his grandchildren, Mark Perfetta (Robin), Joseph Perfetta (Jennifer), Robert Perfetta (Heidi), Gena Biem (Kenny), Steven King (Danielle), Brandon Perfetta, Justine Libengood, Ryan Libengood and Steven "Boomer" Perfetta; his great grandchildren, Hunter, Izabella, Kylie, Chelsea, Kenny, Sophia, Roashell, Stevi, Brooklyn and David; his great great grandchild Lilly; his siblings, Richard Perfetta (Peggy) of Export and Louise Shawley of Slickville; and his sister-in-law Judy Perfetta of Saltsburg. He is also survived by numerous nieces and nephews.
