He is survived by: his wife Cathy Forsythe (Weaver) of Madison; his daughters, Melissa Roy (JS) of West Virginia, Angela Decker (Steve) of North Carolina and Amy Tucker (Robert) of Alabama; and his brothers, Larry and Calvin Forsythe. He is also survived by 8 Grandkids, 3 Great-grandchildren.
