Polk; his sisters, Louise Appleby (Terry), Virginia Miley (Dennis), Dixie Williams (Bill) and Karen Sue Polk; his nieces and nephews, Jason Shultz (Laurie), Dustin Shultz (Amy), Nick DeCarlo (Barbara), Kndra Baxter and Crystal Miley (Howie); and his great-nieces and great-nephews, Draven Shultz, Koen Shultz, Maddox Shultz, Griffin Shultz, Kam Baxter and Madison Shultz.
