He is survived by: his wife Dorothy Slezak (Krivak); his daughters, Karen Platt (Richard) of Myrtle Beach, SC and Dana Slezak of Mercer; and his sisters-in-law, Peggy Slezak (Jerry) of Hermitage, PA, Rheta Slezak (Lester) of Corpus Christi, TX, Rose Marie Russo (Steiner) of Burghill, Carol Augustine (Steiner) of Masury, Jane Steiner (Davis) of Hubbard, OH and Ruth Ischo (Krivak) of Sharpsville. He is also survived by five grandchildren, three great grandchildren and many nieces and nephews.
