He was predeceased by: his parents, Steve Kozakovsky and Mae Knochel Kozakovsky; and his wife Sherry L. Hazlet. He is survived by: his children, James Brian Pudina (Tessa) of Butler, John Matthew Kozakovsky (Amber) of Seneca and Amanda Castile (Joseph) of Oil City; his grandchildren, Angel, Addison, Aidan, Sophia, Jaxson, Archer and Danika; and his siblings, Steve Kozakovsky of Valencia, George Kozakovsky (Lynn) of Connellsville, Rita Kozakovsky of Valencia, Mary Kozakovsky of Kittanning, Florence Pegher (Robert) of Cadogan and Jean Charlton (John) of Valancia. He is also survived by many nieces and nephews.
