He was predeceased by: his parent Dominick Leonard. He is survived by: his parent Audrey Frost Leonard; his children, Kristen and Brian; his siblings, John, Dominick, Cathy, Audrey and Dolores; and his child Ann Marie.

In lieu of flowers, we ask that you donate to organizations aimed at helping others care for their mental health, such as the National Alliance on Mental Illness, the Brain and Behavior Research Foundation, or the Treatment Advocacy Center.
