He was predeceased by: his parents, Ralph Tolley, Sr. and Jean Marlene Eisbacher Tolley; and his sister Stacey. He is survived by: his wife Kimberly Ann Swartz; and his children, Keith Tolley Jr., Kodey Tolley and Krystal Tolley. He is also survived by two brothers, aunts, uncles, cousins, nieces and nephews.

In lieu of flowers, donations may be made to the family.
