He was predeceased by: his parents, Thomas Cook and Nadine Jones Cook; and his brothers, John and Pete Lewis. He is survived by: his close friends, Miranda Gibble of Wilkes Barre, Boston Cook of Myrtle Beach and Annadine Cook of Schuylkill Haven; his grandchildren, Caelen and Jayden Cook; his siblings, Kim Wildermuth, Greg Lewis and Linda Hartshorn; and also, Beverly Walters and Caleb Cook. He is also survived by nieces and nephews.
