He was predeceased by: his mother Eileen M. Vinkovich Kapustik. He is survived by: his father Martin E. Kapustik of Lower Burrell; his wife Chantel Blose Kapustik; his siblings, Katherine Wollitz (Jody) of St. Johns, FL, Michael Kapustik (Pam) of Sarver and David Kapustik (Brooke) of Lower Burrell; his nephews, Jared Kapustik and Josef Wollitz; his niece Lydia Wollitz; his mother-in-law Lee (Dee Blose); his sisters-in-law, LeAnna Blose and Karina Blose (Michael); and his niece Elena Farley.
