He was predeceased by: his parents, Rosemarie "Pinky" Polyak (Moore) and George Polyak; and his brother George Polyak. He is survived by: his wife Helene E. (Bennett); his daughters, Bernadette McGlynn-Polyak and Maria Temme (William); and his sisters, Patricia Crusco (Francis), Marie Sheenan (Joseph) and Mary Jo LeTourneau. He is also survived by 23 nieces and nephews and countless friends and family.
