He was predeceased by: his parents, William C. Polak and Margaret Schugart Polak; his stepfather Richard Klein; his parents-in-law, Michael and Mary O'Toole; his brother William T. Polak; his sisters-in-law, Barbara Polak and Jane O'Toole; and his niece Valerie Polak.
He is survived by: his wife Cathi (Cathi); his daughters, Susan Moore of Dacula, GA, Katie Polak of New York City and Kerri Moore (Kevin) of Conshohocken, PA; his stepson Michael Fenlon (Noel) of Greensboro, GA.; his grandchildren, Matthew, Adam, Peyton, Ever and Micah; his siblings-in-law, Michael, Martin, Patrick (Tracey), Shaun, Dennis (Darlene), Brian O'Toole (Nancy), Maureen O'Toole and Erin Brannon (Ken); and his caregiver Holly. He is also survived by numerous nieces, nephews, grandnieces and grandnephews.
